How to Report an Injury

Upon notification of a work-related injury, the employer is required to notify Midwestern Insurance Alliance. We make the process easy by providing 24/7 incident reporting using an online form.

Although it is acceptable to provide your insurance agent with a copy of First Report of Injury, please do not rely upon your insurance agent to report the injury on your behalf

Reporting a Fatality or Severe Injury to OSHA

  • All employers are required to notify OSHA when an employee is killed on the job or suffers a work-related hospitalization, amputation, or loss of an eye.
  • A fatality must be reported within 8 hours.
  • An in-patient hospitalization, amputation, or eye loss must be reported within 24 hours.
